The forms below are to be used to either request updates to information in a fleet's DOORS account that is not available for the fleet owner to update directly, or to submit information to CARB to complete annual reporting requirements.
| Annual Reporting Forms | Description |
| Responsible Official Affirmation of Reporting (ROAR) | Required for large fleets beginning in 2012, for medium fleets beginning in 2016, and for small fleets beginning in 2018. Must be completed and submitted to CARB by March 1st each year annual reporting is required. |
| Designated Official | The responsible official will use this form to designate a person to be responsible for signing the ROAR and/or other forms required under the Off-Road Regulation. |
| Hour Meter Reading Log for Designated Low-Use Vehicles Operated Inside and Outside of California | Must submit annually by March 1st for vehicles designated as low-use that operate both inside and outside of California. |
| Hour Meter Reading Log for Vehicles Designated as Agriculture | Must submit annually by March 1st for vehicles designated as agricultural (used for agricultural purposes 51-99% of the time). |
| Request for a Change in DOORS Information | |
| Modifying Parent Fleet Assignments | Use this form to add a parent fleet, choose a different parent fleet due to a change in common ownership or control, or split your fleet into fleet portions. |
| Changing Contact Information | Use this form to request changes to your fleet's contact information, including a request for a new username/password. |
| Transfer of Ownership for a Fleet or Fleet Portion | Use this form to transfer ownership of a fleet or fleet portion in DOORS (i.e., after purchase/acquisition of a fleet or fleet portion). |
| Adding a Vehicle with an Existing EIN | Use this form to request to add a vehicle with an existing EIN to your fleet when the previous owner has not sold the vehicle in DOORS. |
| Vehicle/Engine Information of Previously Reported Vehicles | Use this form to request changes to vehicle or engine information originally reported by a previous owner (i.e., the vehicle was purchased with an existing EIN, but some of the vehicle or engine information is incorrect). |

| Reporting Verified Diesel Emission Control Strategies | Use this form to report the installation of Verified Diesel Emissions Control Strategies (VDECS), exhaust retrofits verified by CARB. |
| Owner Information | Use this form to complete the initial reporting when creating a new DOORS account. |
| Non-Diesel or Non-Standard Engines | Use this form to report vehicles with on-road engines that are subject to the Off-Road Regulation, vehicles with engines that were rebuilt to a more stringent emissions standard, or if you have non-diesel vehicles that replaced diesel vehicles. |
| Funded Vehicles | Use this form to report vehicles, engines, or VDECS that were funded through an incentive program and are still under contract. |
| Repowered Vehicles | Use this form to report an engine that was replaced with a cleaner engine. |
| Low-Use Vehicles | Use this form to designate low-use vehicles and submit the engine hour meter logs. |
| Reporting Vehicle and Engine Information | Use this form to complete initial reporting of your vehicle and engine information. |
| Retiring or Selling Equipment | Use this form to report the retirement or sale of equipment in an existing off-road fleet in DOORS. |

Enforcement Advisories
| Advisory 377 | Limits on Idling - effective and enforceable as of June 15, 2008. |
| Advisory 378 | Dealer/Seller Disclosure - sellers in California of applicable off-road vehicles are required to notify the buyer that the off-road diesel vehicle may be subject to retrofit or accelerated replacement/repower requirements. |
| Advisory 381 | Enforcement of Requirements to Use On-Road Certified Engines in On-Road Registered Vehicles. |
| Advisory 391 | Written Idling Policy - as of March 1, 2009, medium and large fleets must have a written idling policy that is made available to operators that idling is limited to 5 consecutive minutes or less. |
| Advisory 394 | Reporting Vehicles - the deadlines for reporting off-road diesel vehicles varied from March 1, 2009, to August 1, 2009, depending on the size of the fleet. However, all fleets were required to initially report no later than August 1, 2009. |
| Advisory 12-02 | Annual Reporting - fleets must annually update and affirm their reporting information is correct. This annual update is required by March 1 of each year and must include a signed statement, called a Responsible Official Affirmation of Reporting (ROAR), affirming that the fleet information is reported is accurate and that the fleet is in compliance. |
| Advisory 12-08 | New Labeling Requirements Effective January 1, 2013. All vehicles newly reported to CARB must be labeled on both sides within 30 days of receiving the equipment identification number. |
| Advisory 12-25 | On-Road Vehicles with Two Engines Were Required to Report by March 1, 2012. (This is an updated version of the old advisory 12-02 B). |
